A review of an evidence-based model to elicit, document and honor people’s goals of care and life sustaining treatment decisions and the statewide improvement initiative that is in progress across Arizona will be highlighted. Focus will be on needed systematic changes and the role of healthcare professionals/providers/lay people to overcome the many barriers through knowledge dissemination and diffusion of innovation.
